New Patient First Visit
We look forward to welcoming you to Beyond Smiles Dentistry. To ensure we provide the highest level of care, we ask that all new patients complete a New Patient Forms prior to their first appointment.
All information provided is strictly confidential and will only be used by your treating clinician.
Please arrive 10 minutes prior to your scheduled appointment to complete the forms.
If you prefer to complete the forms at the home, please advise at the time of booking your appointment so it can be emailed or given to you.
Practice Hours
Monday – Thursday: 8:00am – 5:00pm
Friday: 8:00am – 1:00pm
(Closed on weekends and public holidays)
Parking and Access
We offer reserved patient parking on the day of your appointment. Parking is conveniently located off Owen Street, directly behind the practice.
We are also easily accessible by public transport. Mordialloc Train Station and bus services are located just across Main Street from the practice.
HiCaps Claiming and Payment
We accept all private health insurance and can process your claims on the spot with our HiCaps terminal. Please make sure to bring your private health fund card (physical or digital card) to all your appointments, otherwise your account will need to be paid in full and claimed directly via your private health provider.
